I think plague pandemics are a lot less likely today. Human hygiene has come a long way, as has our ability to deal with pests that would carry it plus our ability to analyze the disease and come up with a cure or treatment for it.
Some new untreatable disease is still possible (e.g., an AIDS virus that can live for 7 days while flying through the air) but I see it as a lot less likely than it used to be.
